COVID-19 Heath & Safety Compliance
COVID-19 has been recognized as a global pandemic by World Health Organization. The Government of Canada, Province of Ontario, public health and University continue to respond to the evolving nature of the pandemic by enacting various health and safety measures.
ALL participants in sanctioned ("A&R") programs, have a role to play in minimizing the risk of exposure to the COVID-19 virus to oneself and to others
By registering, you are confirming that prior to participating in any scheduled activity you:

Understand that the COVID-19 virus is a highly contagious, dangerous and potentially lethal disease, and that all screening processes and self-assessments must be taken seriously, and that you are required to provide accurate information at all times.

Are acting in good faith abiding by all health and safety measures as required by the University and A&R to keep you, your team and others safe. These safety measures include but are not limited to:
- Will wear a face covering over your nose, mouth and chin at all times when required
- Will maintain physical distancing (minimum of 2 metres) from other people whenever possible
- Will follow basic health precautions including frequent hand washing/sanitizing, cough/sneeze etiquette and shared-space hygiene
- Will comply with daily COVID-19 screening practices as required for entry into athletic and recreation facilities and/or programs
- Will stay at home and/or remove yourself from activities/facilities if at any time you believe you are experiencing symptoms and/or have had a possible exposure to the COVID-19 virus
- May be removed from activity and/or environment should you, in the opinion of A&R staff, exhibit any symptoms of the COVlD-19 virus
- Will follow the direction of A&R staff and/or signs and guidance posted throughout athletic facilities, campus spaces and buildings

Will notify your respective Team Captain, Intramural or facility staff immediately if:
- You develop COVlD-19 symptoms;
- You believe you have contracted or been exposed to the COVlD-19 virus;
- You are planning to be tested for the COVID-19 virus;
- You have been advised, by a medical professional, by a public health official, or by using the government of Ontario self-assessment tool, to self-isolate and/or be tested for the COVlD-19 virus

Understand that your personal information may be shared with University staff and/or public health officials on a need to know basis for health and safety and/for public health reasons

Will participate in testing and cooperate with contact tracing when required and will follow all isolation and quarantine protocols, when required
COVID-19 Virus Assumption of Risk, Waiver & Release
PLEASE READ CAREFULLY
By acknowledging and accepting in your IM League intramural registration, you are confirming that you have read, understood and accept the Assumption of Risk, Waiver & Release information and conditions as outlined below.
In participating in A&R sanctioned activity, you understand and confirm you are fully aware that accessing Queen’s athletics and recreation facilities and participating in A&R sanctioned programs carries inherent risks related to COVID-19 virus and its transmission, which cannot be eliminated notwithstanding the care and precautions taken by the University to mitigate against such risks. You understand that the extent of such risks is not fully known and that they include but are not limited to:

coming into contact with objects that may carry and transmit the COVID-19 virus to you;

transmitting the COVID-19 virus to other individuals (e.g. teammates, coaches, managers, other users of the facilities, , etc.);

injuries or illness, including death, arising from contact with the COVID-19 virus, contracting COVID-19 and/or its treatment.
You confirm that, by choosing to access Queen’s athletics and recreation facilities and by participating in A&R sanctioned programs, you are voluntarily undertaking to assume all risk of personal injury, sickness, death, expenses, or other losses that you may suffer as a result, directly or indirectly, of the COVID-19 virus, the exact nature and extent of which are not currently ascertainable or knowable given the uncertain nature of the virus.
Therefore, you, in consideration of being permitted to access Queen’s athletics & recreation facilities and being permitted to participate in A&R sanctioned programs, hereby forever waive, release, discharge and undertakes not to make any claim whatsoever (action, cause of action, demand, suit, or other form of claim) against, Queen’s University at Kingston or its successors and assigns, and any of its or their past, current or future officers, directors, trustees, employees, agents, volunteers or contractors, in respect of any and all damages, losses, personal injury, sickness or death that you may incur directly or indirectly, now or in the future, that are in any way related to the COVID-19 virus and your accessing Queen’s athletics and recreation facilities and/or participating in A&R sanctioned programs. You acknowledge that this waiver, release, discharge and undertaking shall be binding on your heirs, executors, administrators, representatives, successors and permitted assigns. You understand and acknowledge that this paragraph means you are giving up legal rights and/or remedies that may otherwise be available to you.
You acknowledge that the University may, from time to time, impose additional rules, procedures and protocols as it deems necessary or as it may be required to do by public authorities to mitigate against the risk of the spread of the COVID-19 virus and/or transmission of the virus in Queen’s athletic and recreation facilities and in A&R sanctioned programs. You warrant that you will comply with all such rules, procedures, and protocols. You understand and agree that failure to do so will result in your conduct being referred to A&R’s Non Academic Discipline Policy and Procedures, and may result in the suspension of your access to any Queen’s athletics and recreation facilities and/or of your participation in A&R sanctioned programs.
